1. Tingling or “Pins and Needles” in the Hands and Feet
Often the earliest sign, and the one that matters most because of what it signals: the nerves are being affected.
Because B12 maintains the myelin sheath around your nerves, low levels let nerve signaling misfire, producing:
- Tingling or pins-and-needles, typically starting in the fingers and toes
- Numbness
- A burning sensation
- Reduced feeling — not noticing heat, cold, or minor injuries
- In more advanced cases, balance problems and unsteadiness
The pattern is a giveaway: it’s usually symmetrical — both hands, both feet — and it tends to start at the extremities and move inward. This is the symptom to act on fastest, because it’s the one that turns permanent if ignored long enough.
2. Constant Fatigue and Low Energy

Exhausted despite a full night’s sleep — the most common symptom, and the most commonly dismissed.
B12 is essential for making healthy red blood cells, the cells that carry oxygen. Too little B12 means fewer functional red cells (a form of anemia), and less oxygen reaching your muscles and organs. You feel it as:
- Persistent tiredness and weakness
- Breathlessness on mild exertion, like stairs
- Trouble concentrating
- Reduced stamina
- Sometimes unusual paleness, or a faint yellow tinge to the skin
It has a hundred causes — but it’s one of the few a simple, cheap blood test can check directly.
